BUSINESS (CORPORATE) DEBTORS

Important Requirements

The following information must be provided to the Trustee.

Immediately Provide

• All business keys and alarm codes
• Proof of insurance for all business assets
• Location of all bank accounts and financial institutions

Documents Due 10 Days Before the §341 Meeting

• Names and addresses of all officers and directors for the past three years
• Business tax returns filed within the last three years
• Financial statements prepared within the past two years
• Bank statements for the six months preceding the bankruptcy filing
• Copies of all deeds or property ownership documents
• Documentation regarding asset transfers within the past year
• Copies of leases entered into within the past two years
• Completed Business Debtor Form
