ACCEPTABLE FORMS OF IDENTIFICATION

Before the §341 Meeting of Creditors, each debtor must provide one form of photo identification and proof of their Social Security number.

Acceptable Photo Identification

One of the following:

• Driver’s license
• State issued identification card
• United States passport
• Military identification
• Resident alien card

Proof of Social Security Number

One of the following:

• Social Security card
• W-2 form
• Pay stub showing the full Social Security number
• SSA letter verifying the Social Security number

Failure to provide proper identification may result in continuance of the §341 Meeting of Creditors.
